Although I haven’t done the research you have, Tom, here are my thoughts on your questions:
1] President Snow said he expected a ‘reply, some special manifestation’. He did not say he specifically expected a vision or visitation of the Saviour Himself.
2] The Lord manifests Himself at a place and time of His own choosing.
3] Perhaps He chose to appear after the test of President’s Snow’s faith.
4] President Woodruf passed away while on vacation in California: In 1898, following what had become a yearly tradition, President Woodruff, accompanied by President Cannon and others, escaped the summer heat of Utah for a vacation in California. The prophet’s health, however, totally failed him, and on 2 September he passed away in his sleep in the home of Isaac Trumbo in San Francisco, California.

I researched that visit/vision a bit. There is more that the Lord told Lorenzo Snow, which is not readily accessible on the internet. This information, if you think about it, is related to the following questions (which brought me on this search and how I found your site):

(1) Why was Lorenzo expecting such a dramatic reply, a “manifestation” as he called it?
(2) Why did the Lord not appear to Lorenzo in the Holy of Holies?
(3) Why did the Lord appear, instead, to Lorenzo in the hallway outside the Celestial room and Holy of Holies?
(4) What about Wilford Woodruff? Where was he when he passed away?

Those questions have led me on quite the hunt. I have found/gleaned a few answers to those very questions…which certainly made the hunt worthwhile.
